ABOUT OUR PRACTICE:

Galen Medical Group’s Health and Wellness program offers either personal, one-on-one or group sessions with a registered dietitian to help patients manage their personal nutritional needs that could arise from several health conditions. The dietitian will tailor an individualized healthy lifestyle plan for each patient based on his or her nutritional needs, including diet and exercise. 

We have been awarded Pending Recognition by the CDC Diabetes Prevention and Recognition Program to offer their National evidence-based lifestyle change initiative to help alleviate diabetes crisis in the United States. The effort is based on an approach that has been shown to reduce the risk of developing type 2 diabetes by 58%. The CDC recognizes programs that adhere to the agency’s design and performance standards. Our Diabetes Self-Management Education Program is backed by the American Diabetes Association and helps educate patients already diagnosed with diabetes. Our monthly Diabetes Support Group will be available for patients who are living with diabetes along with their friends and/or family members. At these meeting, medical professionals will speak on a variety of topics and there will always be time for questions and answers during and at the end of every meeting. 

Patients are encouraged to check with their insurance provider regarding specific medical nutritional education benefit coverage.

ONLINE REGISTRATION:

This is a new support program open to anyone in the Chattanooga community diagnosed with diabetes, and their loved ones. We will meet monthly to educate, guide and support in a nonjudgmental manner to foster self-management in their diabetes care.  Will strive to empower individuals by providing education, guidance and support they need to live a healthy lifestyle.  We will also promote self-management by boosting self-confidence by helping everyone overcome their own obstacles.  This will be a welcoming, judgement-free environment so that everyone will feel comfortable sharing their own experiences.  We will have monthly meetings that will be focused on one of the multiple problems that people with diabetes may be going through.  We will also have guest speakers from the local medical community come and speak over these topics as well. This is a voluntary program and the participant can choose which session they would like to attend; however, all sessions are recommended.  We encourage loved ones and caregivers to attend with the participants to educate themselves and encourage the participants to engage in a healthy lifestyle.

ONLINE REGISTRATION:

The Prediabetes Program is a course offered by the Galen Health and Wellness program intended to educate people diagnosed as pre-diabetic to lessen their risk of becoming diabetic. An estimated one out of every three Americans has pre-diabetes and is in danger of serious health risks because of the chronic illness.

Left untreated, Type 2 diabetes can result in a number of complications, including blindness, limb loss, kidney failure, nerve damage, loss of sexual function and the inability to fight off infection. Participants that have already been diagnosed with Type 1 or Type 2 diabetes are ineligible to enroll in this program.

Course Details

During a year-long course that meets once a week, our Registered Dietitian can show you ways to:

•   increase your daily exercise routine

•   eat healthier

•   reduce daily stress through problem-solving and coping skills

The course is based on a program created by the Centers for Disease Control and Prevention that can reduce the risk of becoming a diabetic by more than 50 percent.

Who Qualifies

Individuals are eligible for this program if they have a body mass index (BMI) of 25 or more and any one of the following:

•   A1C measurement between 5.7 – 6.4

•   Fasting glucose level between 100 – 125 mg/dL

•   Diagnosed with prediabetes

SERVICES & PATIENT RESOURCES:

Medical Nutrition Therapy is used to treat an illness or condition.  The role of MNT is to manage medical issues and help reduce your risk of developing complications or to prevent or delay disease.  It is not all about dieting.  Instead, it is about managing your special needs through a healthy approach to lifestyle and eating habits.  MNT is provided by registered dietitians (RDs) who are the only health care professionals with nutrition-specific training. 

Through Medical Nutrition Therapy, a Registered and Licensed Dietitian will work individually to help in areas such as nutrition therapy for your special needs, reading food labels, healthy cooking tips, guidelines for eating out, and locating resources. 

What does MNT include?

  1. Doing a comprehensive nutrition assessment to determine your nutrition diagnosis
  2. Creating and implanting a plan using proven (evidenced-based) nutrition practice guidelines
  3. Monitoring your progress

Who qualifies for MNT?

Everyone qualifies for MNT services. You need a referral from your primary care physician, and insurance coverage varies.

Our Diabetes Self-Management Education Program (DSME) is recognized by the American Diabetes Association and we are committed to providing education resources for the patients of Galen Medical Group and their caregivers. We believe that those individuals affected by diabetes must learn self-management skills and make lifestyle changes to effectively manage their diabetes.  The goal is to enable the person with diabetes to delay or avoid the complications associated with this disorder.  Galen Medical Group believes that learning self-management skills is an integral part in accomplishing this goal.
